California Healthy Kids Survey

Every two years, each school district in the state of California is required to give students the California Healthy Kids Survey as a condition of receiving some federal and state monies. The completed survey provides health risk and resilience information to schools, districts, and communities. In April 2008, students in LCUSD grades 5, 7, 9, and 11 will be asked to complete the survey. There are three versions of the survey: elementary school (grade 5), middle school (grade 7) and high school (grades 9 and 11). Written permission from parents is required for students in grade 5 to participate in the survey and the survey is voluntary and anonymous. Parents have the right to request that their child not participate in grades 7, 9 and 11. Students who complete the survey have the right to answer only the questions they want to answer.
